GMSA - Scheduling Request Form

Below is the team sign-up form for GMSA Summer Indoor Session, tentatively set to begin the week of June 14th, and will run through late mid-August. To reserve your place, fill out the information below. Deadline to reserve your team's place in the league is Thursday, May 27th. Details are as follows:

We are offering divisions for Men's, Women's, and Coed teams.
We will be playing 8-to10-games this session. We anticipate men's games on Thursdays and/or Mondays, with coed on Tuesdays and/or Wednesdays. (This could change depending on the number of teams entered in each division.)
Games will be played at the Bancorp South Sports Complex in Southaven, Mississippi (DeSoto County) indoor facility, located near Getwell Road and Goodman Road. (If you haven't been to this facility, it's very nice...it's worth the drive!)
Teams must have a MINIMUM of 11 players on their roster to play...for coed teams, this includes a minimum of three women.
All players will register on-line, just as they do for GMSA outdoor play. Cost will be approximately $30.00 per player. Individual player registration will begin after teams are confirmed.
All GMSA rules regarding registration, rosters and player cards will be enforced, just as they are for outdoor play. (Player cards and rosters will be kept on permanent file at the facility...teams will need to supply a list of jersey numbers before the season starts.)
All GMSA rules regarding uniforms will also be enforced. (All jerseys must be identical in color, brand, and style with permanently affixed numbers...no mismatched shirts, no taped-on numbers.)
All information (cautions, send-offs, 2-minute penalties, and goals) will be tracked.
There will be two referees at each game. Each team will pay one referee $20 cash at the field.
Matches times will be 6:30, 7:30, 8:30 and possibly 9:30 PM, depending on the number of teams entered. (Two 25-minute halves with a running clock.)
PLEASE do not commit to a team unless you are certain you can field a team for every game...we don't want to run into multiple forfeits.

* Enter only one email address. If you have a secondary email address, please list it at the bottom of this page.
** Depending on number and caliber of teams entered for men's and/or coed, there may be only one "open" division, instead of an "upper" and a "lower" division.
(We will have multiple divisions if we have at least four teams sign up for that particular division.)
