You can now download all of the various forms that GMSA uses, right from our webpage.  You will need the latest version of Microsoft Word (Office 97 Version or later) to be able to view these files.  We hope to make them available in other formats as well in the future.

2.. Player Add/Drop/Transfer/Dual-Registration Form (Word Format - 26K)
Use this form to add, drop, transfer, or dual-register a player, either prior to the start of the season or during the season.

3. GMSA 2008-2009 Match Report  (Word Format - 119K) 
Each coach must present the follow MATCH REPORT FORM to the referee PRIOR TO EVERY MATCH (along with your player cards and the game fee).  Each player's name, ID number, and jersey number MUST be accurately recorded on this form PRIOR to the match.
