Many players and coaches have been asking the league for some way to give feedback about referees.  After a few discussions with a few of my fellow referees, committee members, and players, the league has developed the following form as a tool for players to objectively evaluate the referee's performance on a particular game.  If there are any other criteria/questions that you would like to see added to this form, please let me know. 

Keep in mind, this is the referee FEEDBACK form, not the referee SLAM form...it is meant to evaluate a referee's performance fairly, whether it is good, bad, or somewhere in the middle!  Please use it for ALL of the above situations!  I often get questions/complaints about "how players can give feedback about referees," only after what a player feels was a poor performance by a referee.  

As a referee myself, I implore you to utilize this form to give POSITIVE feedback as well.  If you think the referee did a great job, let's hear about it!  Let me put it another way...if you give a referee a glowing recommendation, you are more likely to see that referee working more frequently.  While these evaluations are by no means an "official" method of evaluating referees, they do help in picking up patterns if a referee is having trouble at a certain level, and they will be used to give "cumulative" feedback if and when it is necessary.  Some of you may be surprised to learn (though I actually wasn't too surprised) the overwhelming amount of feedback thus far has been extremely positive...please keep it up!

Pre-Game:  Did this referee (and his assistants) get the match started in an expedient manner?  (Players checked, coin toss, match started at or near scheduled start time?) 10
Positioning:  (Consider closeness to play...was he/she too close to the ball, not close enough, or just about right?  Was the referee at the proper angle to see play clearly, or was the referee often screened away from play?) 10
Hustle:  (Consider overall effort...did the referee sprint when necessary?  Make every effort to be in the best position?) 10
Foul Recognition/Game Management:  (Consider calls AND non-calls...did the referee call fouls appropriately?  Too loose?  Too tight?  Good utilization of the advantage clause?  Were cautions/send-offs given appropriately?) 10
Attitude:  (Did the referee seem to want to be on the field?  Was he involved in the game?  Did he take the time to talk to players when necessary?  Was he willing to address questions without getting into lengthy discussions?) 10
Assistant referees:  (Consider:  Were the ARs actively involved in the game?  Did they accurately maintain a position with the second-to-last defender?  Were offsides calls made appropriately?  Throw-ins?  Were substitutions handled correctly?  Did the AR assist the referee in calling fouls that occurred in his/her proximity?  Did they give appropriate assistance to the center referee as necessary?) 10
Overall impressions/professionalism:  (Consider overall performance...was the game called fairly?  Did the referee keep the players safe?  Did both teams leave the field feeling good about the way the game was officiated?  Did the referee crew treat players respectfully?  Did the referee crew facilitate enjoyment of the match for both teams and any spectators?) 10
